2025 School Registration Form
This form is to be completed by the advisor and will be used to capture basic information for all adults/students in attendance
2025 School Payment Form
This form is to be used by the ADVISER ONLY for payment of a school delegation by check or Credit Card
NCASC Medical Form for Students
A NCASC Medical Form is required for all student delegates. It is a fillable PDF. Hand completed forms will be returned.
Medical forms and check (if applicable) for the school delegation must be mailed in one packet by the adviser
to Bryan Rudolph on or before Saturday - March 1, 2025

call the hotel to make your reservation.
